Waivers

Fill out the electronic waiver(s) by clicking on the appropriate (Adult or Minor) links below.

If you experience any issues with the electronic waivers, you can download a .pdf copy of the waivers here, or arrive early for class to fill out and sign the waivers in person.

PLEASE NOTE: Any participant under the age of 18 will need waivers signed by a parent or court-appointed legal guardian. If the parent or court-appointed legal guardian may not be present, please be sure the participant brings the printed, signed waivers to class.
